Reporting incidents at the CCG

We recognise many staff use hot water bottles while at work for a variety of reasons.

However, there was a recent incident whereby a member of staff was unfortunately burned by the water from a hot water bottle. Therefore, we ask that you only use the microwaveable wheat bags from now on.

In light of this, here’s a timely reminder of what you need to do to report an incident while you’re at work.

You need to log all incidents on the (Datix) reporting system, as soon as possible.

Until your manager is listed on Datix, please select ‘Admin Datix’ on the drop-down menu.

This is the only system you should be using to report incidents.

If you’re a line manager, you will receive specific training to manage incidents reported by your team. Look out for more details on AskHUE coming soon.

You can find a link to the incident report form (on the Datix system) on AskHUE. Go to Policies & resources > Resources & forms > Incident Report Form WECCG (under Forms and templates A-Z).

If you’re unsure what is classed as an incident, check out the following examples:

  • Health and safety – such as the hot water bottle incident above, slips, trips and falls or a road traffic accident while on CCG business
  • Stolen or lost laptops, mobile phones or other equipment
  • Data breaches/information governance incidents – sending a patient’s personal details in an unsecure email
  • Clinical incidents
