Do you have a new starter or leaver in your team?
If you’re a line manager, please remember to complete an IT equipment form for any new starters and leavers.
You will need to fill in this form to request IT equipment (laptop, mobile etc) for any new starters.
You must also complete this form if somebody in your team is leaving the organisation. You need to log any IT equipment due to be returned.
